On September 5, 2015 at about 4PM, OSP Troopers and emergency personnel responded to the report of a single vehicle crash on Highway 101 near milepost 333 (four miles south of Gold Beach).
Preliminary information indicates a 2015 Chevy Tahoe was traveling southbound on Highway 101 when it left the lane of travel and struck a guardrail and then a tree. Upon impact, a section of the guardrail penetrated the passenger compartment and caused severe injuries to the driver.
The driver, Francis T ZAVACKY, age 68, of Steubenville, Ohio, was transported to a local hospital where he was declared deceased. The other three passengers (from Ohio and England) were taken by ambulance to a local hospital for non-life threatening injuries.
OSP was assisted by the Curry County Sheriff's Office, the Gold Beach Police Department, Gold Beach Fire, the Oregon Department of Transportation, and Cal-Ore Ambulance. This is an ongoing investigation and more information will be released as it becomes available.
