On September 22, 2015 at about 2:40PM, OSP Troopers and emergency personnel responded to the report of a single vehicle roll-over crash on Highway 199 at milepost 13 (near Wilderville).

Preliminary information reveals a 2000 Mazda sedan was traveling westbound on Highway 199 when the vehicle drifted off the roadway and rolled over. Upon emergency crews arriving on scene they discovered the driver, Jacqueline IHARA-STAFFORD, age 63, of Cave Junction, was deceased. The passenger, Carolyn S TILLETT, age 61, of Cave Junction, was trapped in the vehicle and was extricated. She was taken to a local hospital for non-life threatening injuries.

Evidence on scene indicates IHARA-STAFFORD was not wearing a safety belt and was ejected from the vehicle. The contributing factors to the crash are still being investigated.

OSP was assisted by American Medical Response, Illinois Fire & Rescue, and the Oregon Department of Transportation. Highway 199 was closed several hours periodically to allow the investigation to occur. The investigation is continuing and more information will be released when it is available.
