On October 22, 2015 just before 10AM, OSP Troopers and emergency personnel responded to the report of a multiple vehicle crash on Interstate 5 near milepost 188 (just south of the Highway 58 junction).

According to Sergeant Hank Duren, a tan 2001 Saturn Sedan was traveling northbound on Interstate 5 when it collided with a black 2015 Dodge Pickup.
